Certification
In this document, "I," "me," and other first person pronouns refer to the Guarantor(s) whether singularly or collectively, who applied for a loan on the property or project referenced above.
I, the undersigned, hereby certify the following:
1. I have applied for a business purpose, commercial real estate mortgage loan from CMG Mortgage, Inc. dba CMG Financial. In applying for the loan, I have completed a loan application containing various information on the purpose of the loan, the amount and source of the down payment, employment and income information, and assets and liabilities. I certify that all of the information is true and complete and that all loan proceeds will be solely for the use of business purposes. I made no misrepresentations in the loan application or other documents, nor did I omit any pertinent information.
2. I understand that most loans require a full review of information provided on the application, including possible verification of the information. I agree that if my loan program does not require full documentation, CMG Mortgage, Inc. dba CMG Financial reserves the right to change the mortgage loan review process to a full documentation review. This may include verifying the information provided on the application with any employer and/or any financial institution.
3. I fully understand that it is a Federal crime punishable by fine or imprisonment, or both, to knowingly make any false statements when applying for a mortgage, as applicable under the provisions of 18 USCA §1014.
4. I provided a verbal and/or written authorization to order a consumer credit report and verify other credit related information in connection with my loan application, including but not limited to any mortgage or landlord reference and any other source of credit as determined by CMG Mortgage, Inc. dba CMG Financial.
5. I further authorize CMG Mortgage, Inc. dba CMG Financial to order a consumer credit report, background report and verify other credit related information in connection with my loan application, including but not limited to any mortgage or landlord reference and any other source of credit as determined by CMG Mortgage, Inc. dba CMG Financial. I understand that the purpose for this order is for CMG Mortgage, Inc. dba CMG Financial to determine my eligibility and creditworthiness for the loan being applied for, as well as for other legitimate purposes associated with my account.
Authorization to Release Information
To Whom It May Concern:
1. I have applied for a business-purpose mortgage loan from CMG Mortgage, Inc. dba CMG Financial. As part of the application process, CMG Mortgage, Inc. dba CMG Financial and the mortgage guaranty insurer (if any), may verify information contained in my loan application and in other documents required in connection with the loan, including accessing a new credit report, either before the loan is closed or as part of its quality control program.
2. I authorize you to provide to CMG Mortgage, Inc. dba CMG Financial, and to any investor to whom CMG Mortgage, Inc. dba CMG Financial may sell or broker my mortgage, and to the mortgage guaranty insurer (if any), any and all information and documentation that they request. Such information includes, but is not limited to, employment history and income; bank, money market, and similar account balances; credit history; and copies of income tax returns.
3. CMG Mortgage, Inc. dba CMG Financial or any investor that funds or purchases the mortgage, or the mortgage guaranty insurer (if any), may address this authorization to any party named in the loan application.
4. A copy of this authorization may be accepted as an original.
5. Your prompt reply to CMG Mortgage, Inc. dba CMG Financial, the investor that purchased the mortgage, or the mortgage guaranty insurer (if any) is appreciated.

Right to Receive an Appraisal: You have the right to receive a copy of any appraisal report obtained in connection with this application. If you would like to receive a copy, please write to us at 3160 Crow Canyon Road Suite 400, San Ramon, CA 94583. We must hear from you no later than 90 days after the date that we notify you of the action taken on your application or that you withdraw your application. In your letter, please provide your name and mailing address, as well as the address of the property on which the appraisal was performed. If you have not previously paid for the cost of the appraisal, you will be required to do so before your request is fulfille
E-Sign Consent. By executing this application, the undersigned consents to having all documents and disclosures provided or made available in electronic form and to doing business with us electronically. The undersigned also consents and agrees to the use of electronic records and electronic signatures in connection with transactions with us instead of written documents and handwritten signatures.
Acknowledgement. Each of the undersigned hereby acknowledges that any owner of the Loan, its brokers, processors, servicers, lending partners, successors and assigns, may (1) verify or reverify any information contained in this application or obtain any information or data relating to the Loan, including but not limited to consumer credit reports and background reports, for any legitimate business purpose through any source, including a source named in this application or a consumer reporting agency, and (2) authorizes the owner of the Loan to release, furnish, provide, and exchange information related to the Loan, mortgage or information set forth herein to potential loan purchasers, and the Lender’s online platform investors (“Authorized Third Parties”).
